Stroke early supported discharge service
A new stroke early supported discharge service (ESD) for Hertfordshire was launched in October 2014. The service means that people who have suffered a mild or moderate stroke can leave hospital earlier because they are being supported at home by a team of experts providing an intensive rehabilitation package for up to six weeks.
There is robust evidence that demonstrates that ESD reduces long term dependency, admissions to institutional care and lengths of stay in hospitals. Up to 40% of patients who have had a stroke can benefit from this treatment. ESD has been integrated across health and social care and is provided by multi-disciplinary teams, including nursing, therapy, social care, the voluntary sector and specialist stroke physicians.
Hertfordshire Community Trust, East and North Hertfordshire NHS Trust, West Hertfordshire Hospitals NHS Trust and Hertfordshire County Council have been confirmed as providers, with the county’s two clinical commissioning groups providing a budget to fund the roll out of the service and to improve patient outcomes.
